Who is eligible for services?

Services available for all members of the community, ages 18 and older. Accepts Medicaid, Medicare, and other insurances. Offers a sliding scale fee for those who are eligible.

We recognize each customer-owner is unique, and as such, the duration of care in our programs is clinically-driven and determined by individual needs.

We provide the highest level of care possible and are committed to ensuring every individual and family we serve receives the support they need to thrive.
